What a Year of GLP-1 Telehealth Really Costs (2026)
Monthly prices are how GLP-1 telehealth programs advertise. Twelve-month totals are what you actually pay. We took ten programs we have reviewed in depth and did the year-one math — first month, ongoing rate, and a realistic 12-month total for compounded semaglutide — using the numbers from each brand’s own pricing pages at the time of our reviews.

The spread is bigger than it looks
Between the cheapest year (Telos at ~$1,138) and the most expensive month-to-month path (TrimRx at ~$3,468), the same molecule costs three times as much. Nothing in that gap is the drug — it’s plan structure: intro rates, prepay discounts, and what happens after month one.
Three pricing traps we keep finding
- The disappearing first month. TrimRx goes from $179 to $299 the moment month two starts. Always ask: what is month four’s price?
- Prepay lock-in. The best rates (bmiMD $99, TrimRx $174) require paying $1,100–$2,100 upfront. Most refund policies won’t rescue you if you quit in month three.
- Dose creep. Programs price the starting dose. If your protocol titrates up, check the higher-dose price before committing — Telos tirzepatide nearly doubles from entry to top dose.
What brand-name costs for context
At our last checks, cash-pay brand-name pricing through one program’s own menu ran Wegovy $1,349, Zepbound $1,069, Ozempic $1,299, and Mounjaro $1,349 per month — while manufacturer direct-pay programs have brought Wegovy to about $349 and Zepbound vials to $299–$499. Compounded programs undercut even those, which is the entire pitch.
