The supplement aisle is the only place in the store where the packaging is legally allowed to imply things the contents can’t deliver. Not lie, exactly — the rules forbid outright disease claims — but imply, suggest, gesture. “Supports immune health.” “Promotes cellular energy.” “Clinically studied ingredient.” Every phrase engineered to sound like medicine while promising nothing a lawyer would recognize.

Here’s the structural fact that reframes the whole aisle: under U.S. law — the Dietary Supplement Health and Education Act of 1994 — the FDA does not approve supplements for safety or effectiveness before they’re sold. Manufacturers are responsible for their own products; the FDA steps in mainly after problems surface. Once you know that, the label stops being information and becomes a document to interrogate. Here’s how a skeptic reads one.

Start with the claim — and find the asterisk

Any “structure/function” claim (“supports joint health*”) must carry the famous disclaimer: This statement has not been evaluated by the Food and Drug Administration. This product is not intended to diagnose, treat, cure, or prevent any disease. That asterisk is the label admitting, in mandated fine print, that the front of the bottle is marketing. A skeptic reads the claim, finds the asterisk, and mentally replaces the sentence with “we are legally permitted to say this about almost anything.”

Red-flag language worth an immediate shelf-return: anything naming a disease (“fights arthritis”), anything promising speed (“melts fat in 14 days”), the word “miracle,” and comparisons to prescription drugs. The FDA and FTC send warning letters over exactly these, and companies making them have already told you how they operate.

Interrogate the Supplement Facts panel

  • Dose per serving, versus the studied dose. This is the big one. An ingredient can be legitimately evidence-backed at a dose the product doesn’t contain. If the research on an ingredient used 3 grams and the capsule has 300 mg, you’re buying the study’s reputation, not its effect. A two-minute search of the NIH Office of Dietary Supplements fact sheets gives you typical effective doses for most major ingredients — that page is the skeptic’s best friend.
  • Proprietary blends. “Energy Matrix 1,200 mg” listing eight ingredients with no individual amounts is a legal way to sell you a pinch of the expensive ingredient buried in cheap filler. The blend’s total weight is disclosed; the recipe isn’t. Treat any proprietary blend as an admission that the individual doses wouldn’t impress you.
  • The % Daily Value theater. 5,000% DV of vitamin B12 sounds heroic; your body absorbs a fraction and excretes the rest of a water-soluble vitamin. Meanwhile, fat-soluble vitamins (A, D, E, K) at mega-doses can actually accumulate to harmful levels. Bigger numbers are a pricing strategy, not potency.
  • The form of the ingredient. Magnesium oxide and magnesium glycinate are both “magnesium” on the front label but differ meaningfully in absorption. Same for chelated minerals, methylated B vitamins, and curcumin with or without absorption enhancers. Cheap forms are how a $6 bottle undercuts a $20 one.

Look for the seals that mean something

Since nobody pre-approves the contents, third-party testing is the closest thing to verification that the bottle contains what it claims and not what it shouldn’t. The seals worth trusting: USP Verified, NSF Certified (and NSF Certified for Sport), and Informed Choice/Informed Sport. These organizations test identity, potency, and contaminants. They don’t test whether the ingredient works — only that it’s actually in there, at the stated dose, without lead or hidden pharmaceuticals as bonus features.

That last part isn’t hypothetical. FDA testing has repeatedly found weight-loss, sexual-enhancement, and muscle-building supplements adulterated with actual prescription drugs — sibutramine, sildenafil analogs, steroids — sold as “natural.” Those three categories are the aisle’s organized-crime district; third-party certification matters most exactly there.

Meaningless decorations, by contrast: “natural” (arsenic is natural), “clinically studied” (by whom? at this dose? did it work?), “pharmaceutical grade” (not a regulated term), “doctor formulated” (which doctor, and did they stay?), and lovely GMP badges self-awarded by the manufacturer.

My honest gripe: the burden this puts on you

Stepping back, it’s mildly absurd that reading a supplement label competently requires knowing a 1994 statute, absorption chemistry, and the difference between four certification bodies. That’s a systemic failure being retailed as consumer responsibility, and I resent it on your behalf. But the system is what it is, and the practical defense is a checklist that takes ninety seconds once it’s habit.

The 90-second skeptic’s checklist

  • Does the front make disease or speed claims? Shelf it.
  • Proprietary blend hiding doses? Shelf it.
  • Is the dose in the studied range? (Check the NIH ODS fact sheet.) No → shelf it.
  • Third-party seal (USP, NSF, Informed Sport) — especially for anything weight-loss, performance, or libido related? Missing → strong lean toward shelf.
  • Do I have an actual reason to take this — a tested deficiency, a dietary gap, a provider’s suggestion? No → the healthiest supplement is the $30 still in your pocket.
  • Does it interact with my medications? St. John’s wort, high-dose vitamin K, and others genuinely tangle with prescriptions — pharmacists answer this question for free.

FAQ

Are supplements just useless then?

No — specific supplements for specific situations are well-supported: vitamin D for documented deficiency, B12 for many vegans and older adults, folic acid in pregnancy, iron when tested low, creatine for strength training. The pattern: a measured gap, a targeted fill, a sensible dose. What lacks support is the shotgun approach of taking twelve things because a podcast said so.

Is a more expensive brand safer?

Price correlates weakly at best. What correlates: third-party certification, transparent dosing, and companies that publish certificates of analysis. Some $12 certified products beat $60 influencer brands on every testable measure.

What does “standardized extract” mean — good or bad?

Generally good. It means the botanical is processed to contain a consistent percentage of the presumed active compound (“standardized to 5% withanolides”). It doesn’t prove efficacy, but it beats unstandardized plant powder, which can vary batch to batch from potent to inert.

Who should be extra careful with supplements?

People on prescription medications, anyone pregnant or breastfeeding, people with liver or kidney conditions, and competitive athletes subject to drug testing — contaminated supplements are a leading cause of inadvertent doping violations. For all four groups, the move is the same: run the specific product past a provider or pharmacist before the first capsule.
